The obvious answer to a gutter under trees is to cut the branch back. Around Hangersley that is the one thing you cannot simply do, because the trees are protected, so the gutter has to be managed instead of the canopy.
Most houses out here are detached and set back among mature oak, beech and pine, and the calls follow the canopy and the seasons. A run that overflows every autumn no matter what. A downpipe gone solid. A ceiling stained after a wet fortnight, which usually means a valley gutter has filled rather than an eaves run.

The work
Under the canopy on the escarpment
The load, and what it turns into. A run under mature broadleaf takes far more than leaf. Oak and beech drop over months rather than weeks, and beech mast, twigs and catkins go in with them. Under pine there is needle as well. Left a season it rots down into a dense silt and sludge that holds water the length of the gutter. Loose material is scooped out, packed material dug out, and the run cleaned to the bottom so the fall works rather than skimmed.
Outlets and downpipes. Under trees the outlet takes the punishment. A single clump of wet leaf over the hole stops a run dead, so each outlet is dug out by hand, the pipe run through from the top and rodded from the shoe where water is backing up, and the gully at the bottom lifted and cleared. Then every downpipe is proved with water while you watch it.
The runs you cannot see. Detached houses out here often have a rear range, a dormer or a flat roofed extension, and each of those makes a valley gutter or a box gutter that is invisible from the ground and floods inwards rather than over the front edge. Those are the ones that show up as a damp ceiling rather than a stain on a wall, so they are cleared as a matter of course and looked at with a camera on a reach pole where the angle is awkward.
Repairs. A run carrying a winter's worth of wet debris is heavy, and weight is what pulls a gutter out of shape. The usual finds are a bracket that has let go, a length sagging so water stands in the dip, and a union whose seal has hardened and started to drip. Brackets are refixed, the length gets a realign and a re-hang to its fall, and a union takes a new seal or is replaced. On the older houses with cast iron it is emptied by hand rather than jetted and the bolt through each joint is checked.
Why it is worth staying on top of. Water over the front edge keeps the brickwork and render underneath wet through the winter and brings the green with it. Water behind the gutter takes the fascia and runs along the soffit until the boards go soft. Soft fascias, damp staining and anything reaching a ceiling get photographed and reported.

What decides the price of a gutter clean
No rate per square metre, because a metre of gutter behind a conifer and a metre on an open gable are not the same metre.
Getting to it. A detached house set back under oak with a valley between two ranges, a chalet bungalow with a dormer in the front slope, and a cottage with a garage and a workshop behind it are three different afternoons, and only the first is likely to want more than a tower rather than a scaffold. A bungalow run comes off a ladder or a pole, a three storey gable wants a tower, and a valley gutter between two roofs wants a proper platform. The work at height rules are why the access is a priced line rather than something absorbed into a rate.
What is in it, and how far it has travelled. Leaf litter lifts out in minutes. Moss washed off the tiles sets into a wet mat that has to be dug out, and silt in a long run behind a hedge is heavier and slower than anything a photograph shows. The number of outlets matters more than the length, because that is where it packs.
Whether it is a clean or a repair. A joint that drips once it is running again, a downpipe blocked at the shoe, a length of plastic that has sagged between brackets: none of that is clearing, and it is the part that decides whether you are booking an hour or half a day.
Comparing quotes? Ask whether the downpipes are being run through or just the gutter cleared, whether the outlets and hoppers are checked, whether it is a clear or a clear and a check for leaks, and who takes the debris away. Two quotes for the same job are usually two different jobs.

Water running down a wall in a downpour soaks the brickwork and finds its way to a ceiling faster than most people expect, so a blocked outlet in a wet week goes to the front of the queue. We clear it, run water through to prove it, and tell you honestly if what you actually have is a broken joint rather than a blockage. If a ceiling has marked rather than a wall, say so when you ring, because that moves the valley and any hidden run to the front of the survey. These properties sit inside the National Park and the Western Escarpment conservation area, and in a conservation area works to a tree need written notice well before any saw comes out, with the authority free to make a protection order in the meantime. So lopping the oak over your roof is not a same week decision, and on plenty of plots it is not yours to make at all.
What that leaves is the gutter itself, and there is more to do there than most people think. Twice a year rather than an annual clear, with the second visit held back until the beech and oak are down. Outlets dug out rather than rinsed, because under this much canopy the outlet is what fails. Downpipes proved with water every visit rather than assumed. Guards considered honestly run by run, since mesh under broadleaf tends to mat over and simply move the blockage somewhere you cannot reach it. Get those four right and the trees stop being the problem, which is just as well, because they are staying.

How often should gutters under mature oak and beech be cleared?

Are gutter guards worth fitting under this much canopy?

At the runs you cannot see. A valley between two ranges, or a box gutter where a flat roofed extension meets the main roof, has nowhere to spill except inwards, so it floods into the building while the eaves gutters look perfect from the ground.
Is the debris under trees harder to clear than ordinary leaf?
Yes, once it has been there a season. Wet leaf, beech mast, twigs and needle rot down into a dense silt that has to be dug out rather than rinsed, and it is heavy enough to pull a run out of fall. Brackets and joints get checked at the same time.
